Fill out the collection card with your name, today's date, and your date of birth.

Rinsing hands in warm water, doing jumping jacks, and swinging your arm
in a circular motion will improve your chance of success.

Clean your middle or ring finger with the alcohol pad.

Twist off the lancet cap.
Firmly press the lancet between the side and middle of your fingertip to
puncture the skin. Extra lancets are provided if needed.

Drip only one large drop of blood onto each circle. Try to fill the entire circle.
Do not touch or tap the collection card. Blood should drip from your finger

Gently squeeze your finger downward to help release more blood.
If blood flow stops, clean another finger and use an extra lancet to finish the collection.

You did it! The hard part is over.
Apply the bandage to your finger.
Let the collection card dry for at least 45 minutes.

Fill out the transport tube with your full name, today’s date, and your date of birth.

Catch your urine in the provided collection cup.

Unscrew the cap of the transport tube to prepare it for your sample.

Use the pipette to transfer urine from the collection cup into the transport tube.

Keep adding urine until you have reached the minimum fill line.
Do not exceed the maximum fill line.

When you have enough urine, screw the cap on tightly.

Once the card is dry, put it inside the biohazard bag.
Put the biohazard bag into the bubble mailer and seal it.
